Adult Membership

$349 per year and $250 for each additional immediate family member

  • 2 Exams (Comprehensive or Periodic)
  • 1 Emergency Visit including Radiographs
  • 2 Routine Regular Cleanings (does not include Periodontal "Deep" Cleanings)
  • Any Radiographs Necessary for Diagnosis includes Panorex Imaging, Bitewings, or Periapicals at each visit
  • 20% OFF Fillings, Crowns, and Extractions

*Payment for the discount plan must be paid in full at the time of registration and is non-refundable. A minimum of 48 hours is required for any appointment changes.
